We are pleased that you took the time to inquire about Dispatch Trucking. We are a family owned and operated Truck Dispatching Company. Unlike most brokers, we are upfront about our fees. Our is approximately $3.00 per hour. This fee covers the dispatching, billing, collecting, and payments.
The following is information that we need from everyone trucking:
General Paperwork:
Trucker's Information Form (Click on this link to download the form).
W-9 Form (Click on this link to download the form). Keep in mind that if you do not provide an accurate W-9 form to someone that you are working for, they have to by law, hold 31% of your gross pay.
Proof of enrollment in a random drug testing pool. If you need help choosing a random drug testing pool, click here to see a list of companies MNDOT compiled. Participation in a random drug testing pool is required by law for all ITOs and MTOs. If your company has a compliance review and you are found not to be participating in a random drug testing pool, you will receive an acute violation and a fine of at least $2,000.00. Copy of your most recent DOT Inspection.
Signed service agreement. You will need to contact Jennifer to get a copy of this.
Insurance Information:
Before you can work with Dispatch Trucking, we will need to have a Certificate of Insurance for Liability and Workers Compensation on file. Dispatch Trucking requires both General Liability and Auto Liability. Your insurance company must list Dispatch Trucking as an additional insured. Download a sample certificate to provide your insurance agent. If you are an ITO who would like to waive out of Workers Compensation, please return the following waiver to Jennifer.
In addition to the above information, all ITOs need to provide:
Copy of the front and back of your drivers license.
Copy of the cab card of your truck with your company name and truck number written on it.
Copy of your paid Highway Heavy Tax Form. If you have a new truck and have not had to pay the Highway Heavy Tax, your Application for Title can be used instead.
In some cases, specific contractors require their own information to be provided. Please keep in mind that the above information is required. Not providing any of the above information could prevent you from work, and will cause delays in payment. If you have any questions regarding any of the requirements, please contact Jennifer. If for some reason you are not able to provide some of the above information, you will need to provide in writing what it is you cannot provide, and the reason.
